"The Girl Who Was Death" doesn't feel remotely like it would have fit in on DANGER MAN--even with those final two color episodes if they signified a slight change of tone. It feels like an exaggerated PARODY of DANGER MAN to come degree... but even more of the other spy shows you name, and especially, to me (as I mention in the review) of THE AVENGERS. But above all, it feels like an episode of THE PRISONER! Sure, it's an atypical one that doesn't follow the standard format, but it certainly has the same surreal, psychedelic touches found often on that show, yet never on DANGER MAN. So I have trouble it was written for anything other than THE PRISONER. Danger Man was relatively realistic, and did not go in for the "superhero secret agent saves the world from the mad scientist"-type plots. My impression is that the episode was a spoof of 1960's spy-fi movies and TV series (Bond, Matt Helm, U.N.C.L.E., etc.) in general. <br /><br />I am not so skeptical about the possibility that The Prisoner was John Drake, but, as you say, McGoohan had to deny it, since he did not own the rights to Danger Man. I know I'd read rumors about a scene like you describe years ago (though I'd forgotten the episode) and I listened carefully, but like you say, it's not there. I suspect that's apocrypha.
